Required Exclusion (Section 27-8.1 of the School Code): Students in grades requiring submission of the Certificate of Child Health Examination Form are required by law to be excluded from school if the Form, or a Religious or Medical Exemption, is not received by October 15th of the current school year.
Immunization Data Reported to ISBE
The District annually reports student immunization data to the Illinois State Board of
Education as required by Illinois School Code 105 ILCS 5/27-8.1. This data is available to
the public for review through requesting it directly from Illinois State Board of Education
(ISBE). District 26 policy regarding the implementation and regulation of student
immunizations can be found in the Board of Education’s Policy Manual located on the
District 26 website under Policy 7:100 Health Examinations, Immunizations, and Exclusion
of Students. -

1. Faxed to a secure fax at (847) 516-9705.
2. Handing in person to your building nurse.

If your child requires medication or specialized healthcare monitoring during the school day, the appropriate signed physician forms must be filed in the health office.

Students transferring to Illinois from another state, another country, or home school must provide:
- A physical examination completed within one year prior to enrollment in an Illinois school.
- Proof of all required Illinois immunizations, including the meningococcal (MCV4) vaccine when applicable.
- A completed eye examination performed by a licensed optometrist or ophthalmologist.
Out-of-state health forms are accepted for students enrolling in an Illinois school for the first time, provided they include all required components of the Illinois Certificate of Child Health Examination.
Transfer students have 30 days from their enrollment date to meet Illinois health requirements.
Returning Illinois Students: Students who previously attended an Illinois school, moved out of state, and later return are only required to submit a new physical examination at the state-required grade levels (Kindergarten and 6th grade in District 26).

Please review these guidelines before sending your child to school in the morning. Keeping sick children home protects our entire school community.
Keep your child home if they have:
-
Fever: A temperature of 100°F or higher. Students must remain home until they are fever-free for 24 hours without the use of fever-reducing medication (such as Tylenol or Motrin).

Vomiting or Diarrhea: Your child must be free from vomiting or diarrhea episodes for a full 24 hours before returning to class.

Unexplained Rash: Any undiagnosed or spreading rash must be evaluated and cleared by a licensed healthcare provider before the student may re-enter school.
Reporting an Absence: If your child is staying home due to an illness, you must notify your school’s individual attendance phone line by 8:30 AM.
